Ultima V: Lazarus is a remake of Ultima V: Warriors of Destiny. It is a mod for Dungeon Siege developed by a team led by Ian "Tiberius Moongazer" Frazier over several years. Ultima V: Lazarus had the following main releases:
None of the download mirrors hosting the alpha demos are still around. As for the final release, the only mirror from the downloads page that still works is the one at TheHawk's Nest. It can also be obtained from the relevant project entry at The Ultima Codex.

There's a Facebook thread in which Frilly Wumpus (via Wyrdweb) provides instructions on how to get Ultima V: Lazarus working with the GOG version of Dungeon Siege. Frilly Wumpus' instructions are quoted below in case they ever disappear:

"Ok, so it wasn't as easy as adding a registry key. I guess things have changed with Windows 10 and I couldn't get that to work. But one thing that I made sure of when I created the installer is that you would still be able to manually create the shortcut if you needed to. Everything in the zip file is exposed and not bundled up inside the Setup.exe file. (That was an issue with Lazarus)
"To get set up, do the following:
- unzip the u6 project zip file to the c: drive (c:\u6_project_1_1)
- in the top level of the unzipped directory (c:\u6_project_1_1), right+click an empty space in the file explorer (not a file) and select New -> Shortcut
- in "Type the location of the item", put: "C:\GOG Games\Dungeon Siege\DungeonSiege.exe"
- Next
- In the name, put "Ultima 6 Project"
- Finish
"Next, right+click on the newly created shortcut and bring up "Properties"
"In the "Target" box, replace the contents with:
"C:\GOG Games\Dungeon Siege\DungeonSiege.exe" map_paths=!"c:\u6_project_1_1\resources" res_paths="c:\u6_project_1_1\resources" user_path="c:\u6_project_1_1"
"(Note that the above is all on the same line. If you are using different directory paths, replace as necessary)

Several mods have been made for Ultima V: Lazarus, which is itself a mod. Several of these are listed:
The Enhanced Graphics Mod was developed by XiDragon and replaces some of the models and 3D textures with improved versions. The original website has disappeared and this mod is hard to find nowadays. See below for info and downloads.

DOUG the Eagle Dragon wrote an anti-walkthrough for Ultima V: Lazarus, and it is accompanied by the following videos:
Amazing Dragon, the same one who wrote The Lost Vale fanfiction, has a YouTube channel with dozens of ten-minute videos of Ultima V: Lazarus in action, recorded in 2009.
A page about Ultima V: Lazarus with "talking points" and old download links and another about the Ultima V: Lazarus v. 1.1 patch used to be part of the old Ultima Web Directory at Dino's Ultima Page. Pretty much all the links are now broken and these pages remain only for historical value.
Nazghul was a game engine resembling the Ultima 5 engine, and its goal was to do what the U5 engine can, and better. The actual game was in no way related to Ultima - the only connection to Ultima is the engine being based on that of Ultima 5.
Nu5 is to be an Exult-inspired engine for Ultima 5.
Ultimapper 5 is a tool that complements the game by adding a HUD and an automapper. It also has other features such as quickfight.

Ultima 5 Redux is a set of C# libraries using the original game files, as well as a new game engine based on Unity3D. The project was announced in September 2019 and the project is moving in the direction of an isometric 3D perspective. See the project's WebDB2 entry to learn more about it.
