Put the Glyph on the altar, put it back in inventory and take the teleporter

back down.  Go back up the teleporter and put the Ankh on the altar.

You will be prompted for the Mantra.  Type it in and the Ankh goes up

in the tornado all by itself.  When it's done you have a 3-piece set!
